Oceanic Announces Regulator First Stage Recall/Retrofit

Oceanic has been notified by its supplier (ROMI, Ent.) that they have received isolated reports of DX type Regulator First Stages exhibiting uncontrolled flow of air to the Second Stage.
Evaluation has shown that a specific batch of HP (High Pressure) Seats contained material that can deteriorate and fail to properly perform its mechanical sealing (shut off) function, causing an uncontrollable free flow that could result in serious injury to or death of the user.
In cooperation with the U.S. CPSC (Consumer Product Safety Commission), ROMI is initiating a voluntary recall of all affected CDX5 and FDX10 (diaphragm) type First Stages.
The affected First Stages include those that were shipped to Oceanic retailers from the factory between May 1, 2006, and Nov. 15, 2006. Other model DX type First Stages, such as the CDX, DXi, DX3, DX4 and TDX5, that received annual service between May 1, 2006, and Oct. 22, 2007, may also be affected.
NOTE: Due to the random nature utilized to assign SNs (serial numbers) to First Stages, there is only a small quantity of affected units within the large range of SNs. Since the extent of listing the actual SNs of all affected units would be impractical and not user friendly, Oceanic has created a data base on the Oceanic Worldwide web site that product owners and retailers can easily access to determine whether a specific First Stage is affected and if it has received the retrofit.

If you have an affected unit, you must discontinue further use of it and take it to the Oceanic dealer where you purchased it or a local authorized Oceanic service center for a component retrofit at no charge.

It has come to our attention that some S.O.S. units (PN 394157] and 394057) sold since Sept 2002 may have a faulty component.
The component is known as an "elbow," and is used to transfer air from the BC to the safety tube. Some elbows have
exhibited signs of degradation leading to cracking and/or breakage.
If this were to occur, air may not successfully transfer from the BC to the safety tube, thus preventing the tube from inflating.
Furthermore, if a diver were inverted, making the attachment point the "high spot," air could also escape from the BC.
The leak may prevent the BC from "fully inflating." Although the BC will still
hold air when vertical, some air will
escape at the bottom and the BC will provide less buoyancy.
To date, there have not been any reported accidents or injuries.
Aqua Lung would like all owners of S.O.S. units to carefully inspect them. Elbows that have the potential to degrade are easily
identified by a molding gate mark located
on the outer bend (see photo, right.)

If you have a unit with such a gate mark, we ask that you stop using the product. Detach it from the BC and return it to your nearest Aqua Lung Dealer. You may find your nearest Aqua Lung Dealer by using the dealer locator at

Innovative Scuba Concepts Recalls Diving Regulator Components Due to Drowning Hazard
WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.
Name of product: Swivel for a Scuba Regulator
Units: About 170
Importer: Innovative Scuba Concepts Inc., of Colorado Springs, Colo.
Hazard: The swivel, which is attached to a diving regulator, could separate while diving. This will result in a complete and sudden loss of the diver’s air supply, causing the diver to engage in emergency ascent procedures. This poses a risk of decompression sickness due to rapid ascent, and air embolism or drowning if the diver panics or the emergency ascent procedure fails.
Incidents/Injuries: The firm has received one report where the swivel separated during a dive and caused the diver to ascend using the buddy breathing technique. No injury was reported.
Description: The recall involves the HO110 Swivels sold as an aftermarket regulator component. There are no markings such as a date or production code embossed on the unit. Contact Innovative Scuba Concepts for information on where the recalled units were sold.
Sold at: Specialty retail dive stores nationwide from January 2006 through March 2007 for about $40. They also could have been installed by a dive shop regulator technician.
Manufactured in: Taiwan
Remedy: Consumers should stop using regulators with the swivel attached and contact the dive store where purchased for a refund.

Apeks regulator

There may be a potential problem with the yoke clamp screw on any yoke clamp type regulator with the serial number starting from 7010001 – 7053528. All Apeks dealers have been made aware of this issue.
Please return your Apeks regulator or just the yoke clamp screw to your nearest Apeks dealer for inspection. If you need assistance finding your nearest Apeks authorized dealer, please use the dealer locator found at www.aqualung.com. Do not dive with the regulator until it has been inspected.

ERIS Announces Voluntary Recall of EPIC Wrist Watch Dive Computers

Safety notice affects AERIS EPIC Wrist Watch dive computer.
February 12, 2007- San Leandro, CA - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), AERIS of San Leandro, California, is voluntarily recalling AERIS EPIC Wrist Watch Dive Computers. AERIS has been notified by its supplier (Pelagic Pressure Systems) that they received isolated reports that two dive computers that use the same software program as the EPIC locked into the Gas Switch-to screen and did not return to the Main Dive screen when the divers switched the units from Gas 1 to Gas d2 during dives when they were only using one Transmitter, and Pressure associated with Gas 2 was being provided by an Analog Gauge.
Affected are AERIS EPIC Wrist Watch Dive Computers having serial numbers 1 through 712 that were sold between October 2006 and February 2007, and are being used for diving activities that include use of multiple gas mixes not associated with multiple transmitters.
Testing and evaluation has shown that this can occur if the control button used to switch Gases during a dive is depressed too long (4 seconds instead of the 2 seconds required to perform the operation) or it is pressed multiple times for 2 seconds when a Gas Switch operation is performed when there are multiple Gases (FO2 mixes) but not multiple Transmitters. Engineering programmers have modified the software of the EPIC to prevent the Switch-to screen from locking on the display.
AERIS asks that all owners of affected AERIS EPIC Wrist Watch Dive Computers return them to the Authorized AERIS Dealer from which they were purchased for a software upgrade at no charge.

Scubapro USA announced a recall Oct. 2 of its Scubapro Mark 20 first-stage regulators

Name of Product: Scubapro® MK 20 First Stage Regulators
Units: About 48,500 units
Manufacturer: Scubapro® USA, of El Cajon, Calif.
Hazard: Over-tightening of the Yoke or Din retainer during annual servicing of these regulators could result in a stress crack and failure of the regulator. If this occurs during a dive, air supply could be interrupted, posing a drowning hazard.
Incidents/injuries: No injuries or underwater incidents have been reported. Scubapro® USA has received eight reports of units that failed after being serviced.
Description: The recall involves first stage regulators that are marked with the Scubapro brand and identified as the MK20.
Sold at: Authorized Scubapro® dealers nationwide from January 1996 through December 2001 for about $260.
Manufactured in: Italy and United States
Remedy: Scubapro® USA has been providing a service upgrade kit via the authorized dealer network since May 2005 which contains a Yoke or Din retainer that prohibits over-tightening during annual service. Consumers can easily identify if the service upgrade has been completed by counting the vents on the plastic saddle located on the regulator. If the service upgrade has not been completed, the consumer should bring the regulator to any authorized Scubapro® dealer to have the service upgrade completed free of charge.

SCUBAPRO is conducting a voluntary recall of the X650 second stage regulator.

A manufacturing error could cause the main housing of some regulators to change shape over time, and the cover and diaphragm to become dislodged. If this occurs, air flow will be interrupted and the regulator will no longer function.
Only X650 second stage regulators purchased from authorized Scubapro dealers from November 2004 through May 2006, are affected by this recall. You can determine if you have one of these units by looking for a visible raised bump located on the right side of the regulator between the letter “C” and where the mouthpiece attaches to the regulator body.
At Scubapro, safety comes first and we ask that you stop using the X650 second stage regulator immediately. Replacement Program

Suunto Issues Important Computer Safety Alert (Added 06/20/06)
IMPORTANT SAFETY NOTICE REGARDING D9 AND D6 DIVING INSTRUMENTS
(DNW) - Suunto Oy today announced an important safety notice regarding its D9 and D6 diving instruments. This global decision involves D9 and D6 instruments that have been delivered to distributors and retailers, as well as product that has been sold to consumers.
Quality and safety are paramount concerns for Suunto Oy. Our testing program for the D9 and D6 product by Suunto employees and independent divers all over the world has been extensive. Despite this, Suunto Oy has identified a software bug in the D9 and D6 instruments. The software bug may cause the D9 and D6 to incorrectly track dive time on rare occasions, potentially causing a risk to the diver.
The products affected are:
D9 serial numbers 62102582 and below
D6 serial numbers 62103693 and below
The software bug may activate when the D9 and D6 automatically recalibrate to keep the correct time, or the user manually updates the seconds in set mode. Suunto approximates that there is 0.6 % (a six-tenths of one percent) chance that the software bug may appear during a dive. When the dive time is incorrect, every fourth second is not registered in the dive time and surface interval time, i.e., dive time goes 25% slower than actual time and tissue loadings are recorded at 25% less than actual loadings. As a trained diver you understand that this could affect your decompression requirements and that the length of a dive is one of the key factors in determining those requirements.
Even though the D9 and D6 will function correctly approximately 99.4% of the time during a dive, Suunto wants to eliminate the very small possibility of recording the dive time incorrectly. Therefore, Suunto is requesting that all D9 and D6 shall be returned for a software update that eliminates this issue. To date, there have been no known injuries related to this issue.
Action that YOU should take
· Suunto strongly recommends that you should not use the D9 or D6 for diving without backup instrumentation before the unit has been updated with the latest software. Please bring your D9 and/or D6 to your nearest local authorized Suunto retailer for the software updated as soon as possible.
· Please contact your local authorized Suunto retailer – for contact information please visit our web site www.suunto.com You will then be advised on how to bring your D9 and/or D6 to the retailer for a software update. Once the retailer receives your D9 and /or D6 they will then forward your unit to the Suunto Service Center for the software update. You will be advised by the retailer the approximate turn around time that the process will take.

Versa Pro revision 2A Dive Computers (Added 05/11/06)

May 3, 2006 – San Leandro, CA – In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Oceanic Worldwide of San Leandro, California, is voluntarily recalling Oceanic Versa Pro revision 2A Digital Dive Computers. While operating in the User Selected Digital Gauge Mode, displayed Elapsed Dive Time can be in excess of actual elapsed time.
Oceanic has received a report of two Versa Pro revision 2A units that experienced the offset time displayed while operating in User Selected Digital Gauge Mode. No injuries have been reported.
Oceanic Versa Pro 2A Dive Computers subject to the recall are only the revision 2A units which have serial numbers 12000 through 18176 which is located on the side of the module and can be viewed if after the front button is depressed to activate the unit, it is held depressed until the Diagnostic Mode screen completes its countdown and the Serial Number screen appears displaying the module’s SN and software firmware revision (r2A). The unit will shut Off when released.
Authorized Oceanic dealers sold Versa Pro 2A modules, that were manufactured between March 21, 2003, and February 7, 2005, in wrist and console mount configuration during March 2003 through April 2006 for between $429 to $639, depending on the configuration with other instruments.
Consumers should immediately discontinue further use of Versa Pro 2A Dive Computers for User Selected Digital Gauge Mode operation (as a Depth Gauge/Timer) and take it to any Authorized Oceanic Dealer, or return it directly to the factory for an Upgrade to Versa Pro revision 2B.

HTM Sport S.p.A., Manufacturer of Dacor and Mares Dive Computers, has discovered that during production, a small number of Mares M1 (and Dacor Darwin) dive computers were improperly calibrated, resulting in erroneous calculations displayed regarding "No Dec Time", Deco Time", "Air Time", "Desat Time" and "No Fly" times.

Cressi-sub Buoyancy Compensators (Added 02/28/05)
Cressi-sub, in cooperation with the United States Consumer Product Safety Commission, is advising Consumers to stop using Cressi-sub Buoyancy Compensators with model numbers J107, J113, J115, J119 (the numbers appear on the top right hand corner of the unit on the shoulder), immediately unless otherwise instructed. Slow leakage from the shoulder exhaust caused by expansion of an internal cable housing may result in slow deflation. This could result in unexpected buoyancy problems posing a potential risk to the safety of the diver. Although no incidents have been reported the company is nonetheless issuing a product recall for the above model numbers.
Buoyancy Compensator devices provide buoyancy control for scuba divers by allowing them to inflate or deflate their devices, thus changing the diver's buoyancy. A buoyancy compensator is a bag that inflates and deflates depending upon your buoyancy requirements. In this way, the buoyancy compensator allows you to maintain neutral buoyancy, which is an important part of scuba diving. The units were sold from January, 2003 until approximately September, 2004 and sold at maufacturer's suggested retail prices between $400 and $700.
Some of the devices with the above model numbers have already been retrofitted. To determine whether your device needs to be retrofitted look for a black retaining ring on Buoyancy Compensator devices. The ring surrounds a yellow Inflate button on the inflation mechanism and can be found adjoining a gray cover assembly (bypass case) which screws onto the inflator. Devices which have already been retrofitted, and which therefore do not require corrective action, are those with yellow retaining rings and black cover assemblies.
You can return the product to a Dive Shop Authorized to sell and service Cressi-sub products ("Authorized Dive Shops"), such as the shop from which you may have purchased the product, to have the product retro- fitted with a replacement hose at no charge to you. Dacor Darwin Air and Darwin Console Dive Computers (Added 12/13/04)
If you have recently purchased a Dacor Darwin Air or Darwin Console
Dive Computers please read the following safety notice regarding the
recall of certain computers.
HTM Sports S.p.A., Manufacturers of Dacor Diving Computers, has
discovered that during production, a small number of Dacor Darwin
Dive Computers were improperly calibrated, resulting in erroneous
calculations displayed regarding "No Deco Time", "Deco Time", "Air
Time", Desat Time" and "No Fly" times.
The specific products that may have this defect are in the following
groups:
Darwin Air Dive Computers Serial Numbers between: 3265 and 3415
Darwin Console Computers Serial Numbers between: 8503 and 8592
To locate the serial number of your Darwin Air or Darwin Consol:
1. Find the watertight batter compartment cover located on the
top of Darwin Air or the left side on the Darwin consol.
2. Unscrew the watertight cover by turning 90deg counter
clockwise
3. Remove the batteries and the put them back in after a few
seconds the serial number of the instrument will appear on the
screen.
If you own one of the Darwin Computers with a serial number listed
above DO NOT USE IT!
Diving with an improperly calibrated dive computer will provide
inaccurate No Decompression (no stop) Time, Decompression Time,
Desaturation Time, and No Fly Time, and could result in serious
injury, including Decompression Sickness due to Omitted or
Insufficient Decompression!
Immediately take your dive computer to your Authorized Dacor Dealer
for repair. You should expect to receive it back in 4 - 6 weeks.

Seaquest/Aqua Lung BCs with Powerline Inflator (Added 12/13/04)
Airway Ribbed Hose Replacement
Aqua Lung has received reports that some of the ribbed hoses
on the Powerline Airway have been tearing at the top
near the dual exhaust valve. The problem has been traced to an
error in the molding process from our supplier that occurred
over a three-month period between 2001 and 2002. The best
way to confirm whether or not you have one of the questionable
hoses is to look at the serial number on the lower unit of
the Powerline. If the serial number falls within the range
of M48H to M52H (M48H, M49H...M52H), or M01I to M08I
(M01I, M02I...M08I) and is the original hose, then it should get
replaced. This affects both SeaQuest and Aqua Lung BCs utilizing
the Powerline inflator.
If the ribbed hose were to tear near the top, the BC would not
be able to hold air and a loss of buoyancy would occur. This
could result in the need for the diver to ditch their weight pockets
or rely on their buddy for assistance. In extreme cases it
could result in result in panic, injury or death.
If you have a spiral hose that falls within the serial number
range, please have the hose replaced by this authorized Aqua
Lung retailer.
Oceanic Reliant B/C Inflator Recall
Oceanic has been informed by its supplier that they have received isolated reports of BC inflator mechanisms exhibiting continual flow of air into the BC after release of the power inflation button. Affected are Oceanic Reliant model inflators that are fitted on various models of Oceanic's BCs that were sold between February 1, 2004, and June 24, 2004. BCs sold prior to February 1, 2004, do not have these inflators and are not affected.
Testing and evaluation has shown that the power inflator button may stick in the depressed position. Engineers have designed new replacement parts that will prevent the button from sticking.
Oceanic asks that all owners of Oceanic BCs that have Reliant inflators return them to an authorized Oceanic Dealer for a retrofit at no charge.

Sea&Sea is recalling its DX-3100 Aquapix amphibious camera due to the possibility of a crack appearing in the battery compartment door that may lead to water leakage...
According to Stephen Ashmore, President / Tabata USA - Distributors for Sea&Sea in the U.S., Caribbean and Central America, "Although we have only received a limited number of these reports, even the remotest possibility of this condition appearing has prompted Sea&Sea to take affirmative and positive action, and therefore we would like to request that you return your DX-3100 Aquapix to the following address for testing and any necessary upgrading if within these corresponding serial numbers."

"Although all our products are pressure tested before leaving the factory, our Research Department determined that the reported cases resulted from micro cracks, caused by physical stress after a certain period of time, that formed in the battery compartment door.

Light & Motion Safety Recall on SunRay Mod Light
Effective immediately, Light & Motion announces a Safety Recall on the Sunray Mod Light.
There have been a few incidences in which the light inadvertently turned on while unattended. It has been determined that if the light accidentally becomes partially flooded, it is possible for the light to turn on by itself. If you suspect the light has been flooded, it is important to unscrew the silver bezel and remove the light bulb from the socket.
Light & Motion is taking the cautious route and issuing a Safety Recall and is requesting that the lights be sent in for upgrading. Please contact the company to obtain an RMA number to facilitate the servicing...
