Oceanic - OC1 Firmware Update (Added 6/22/10)

Important 1C Firmware Update from Oceanic.

Units affected are the OC1 model containing version 1A or 1B firmware.

In response to input received from those who have now experienced ownership and use of the OC1, Oceanic has corrected or modified the following items to improve the OC1's operation:

*** Eliminated premature entry into Decompression mode upon descending on NORM Nitrox dives.  This occurred when the dives were conducted after setting FO2 Gas 1 for Nitrox (a numerical value) followed by the M button being pressed to step back through the Plan Mode's Pre Dive Planning.

*** Corrected Temperature displays that appeared only as 0 degrees on the Dive and Log mode screens when the associated dive was activated by the wet contacts upon descent while in Watch mode.  Temperature displayed as expected during other conditions.

*** Inserted the far left digit of the Watch Countdown Timer which remained blank with Hour Format set for 24 Hour.

*** Modified the Set Watch Daily Alarm and Countdown Timer, and Free Mode Countdown Timer, functions to make them to improve ease of access and saving of the selection.

    UPDATE PROCESS

In the event that you have experienced any of the above, or you want to be sure they don't occur, you can upgrade your OC1's firmware (operating software program) to newly released revision 1C.02 using the Check for new Firmware Update feature of the latest OceanLog PC interface program (version 2.2.7).  There is no need to return the unit to Oceanic.

SeaCure - Sport Custom Mouthpiece (Added 6/22/2010)

SeaCure is recalling all Sport I, Sport II, and Sport III Mouthpieces that are being used on Atomic regulators (2005- present), Zeagle regulators (all models/years) or Cressi-Sub Ellipse regulators (all years). The SeaCure Sport Custom Mouthpieces with yellow inserts may be clear, blue or gray in color.

The SeaCure Sport Custom Mouthpieces with yellow inserts were discontinued in 2006 and replaced by the SeaCure Hi-Flow Custom Mouthpieces.  At the time, it was SeaCure’s information that very few of the Sport units were left in retailer’s inventory.  However, SeaCure recently learned of three incidents involving two Sport models that were affixed to an Atomic regulator that was manufactured in 2005 or thereafter and to a Zeagle regulator.

Investigation of these incidents revealed a sizing problem with certain Sport custom mouthpieces and the Atomic, Zeagle and Cressi-sub Ellipse regulators.  In order to place the Sport mouthpieces with yellow inserts on these regulators, they have to be stretched to the point that the yellow insert could detach from the wall of the mouthpiece and move.  This has the potential to cause a problem for the diver because the movement of the insert into the second stage of the regulator could make breathing difficult.  If the mouthpiece does not contain a yellow insert, this is not a potential problem because you have the updated SeaCure Hi-Flow mouthpieces.

    1. Stop using these SeaCure Sport custom mouthpieces immediately if you have an Atomic regulator (2005- present), Zeagle regulator (all models/years) or Cressi-Sub Ellipse regulator (all years).

    2. Contact SeaCure for a free replacement Hi-Flow custom mouthpiece 800-428-9494

OMS Wing Recall (Added 12/28/2009)

Ocean Management Systems Inc. (OMS) ANNOUNCES Safety Replacement OF
BCS SEAL RING (Item # BCA 500)

Ocean Management Systems, Inc. (OMS) has notified the U.S. Consumer
Product Safety Commission (CPSC), that they are voluntarily recalling
approximately 19,790 Sealing Rings BCA-500. Molding variations in this
part as used in the OMS Buoyancy Control System (BCS) could cause
cracking or breakage of part, resulting in rapid loss of buoyancy,
creating a potential drowning hazard. While this variation is not
present in every Seal Ring (Item # BCA 500), it would be difficult to
determine in the field which Seal Rings are suspect. Therefore they
are requiring that all Seal Rings be updated with new assemblies. All
NEW seal rings are equipped with reinforcement at critical areas. 19
failures have been reported of 19 failures. These failures were
discovered during inspection, with no reports of injuries or death.

For further instructions or if you have questions, you can call OMS at
1-8845-692-3600, email at recall@omsdive.com or look on the website
for further information at Ocean Management Systems Product Recalls & Advisories http://www.OMSdive.com

http://www.omsdive.com/downloads/BC%...%2012-09_2.pdf

The seal ring is used on the dump valves as well as the inflator hoses. For a standard wing, you need at least three of these - 2 dump valves and 1 inflator hose. If you plan to do this yourself, you really need the tool mentioned on the instruction page.

At the same time, OMS also announced a factory refurbishment plan for all their BCS's. Basically get a refurbished BCS for $127 for the single bladder/small wings. This includes outer skin replacement if needed.

Mares NEMO Air quick-disconnect o-rings the subject of this important notice.

(Added 8/17/09)

The following was issued by Mares Management this afternoon:

Please be informed that we have discovered a quality issue on the O-ring assembled on the Nemo Air quick connector.

Under certain circumstances, the O-ring can fail and consequently Nemo Air may start leaking through the Quick Connector. As a result, there is a continuous, albeit slow, loss of breathing gas and Mares has decided to issue a voluntary product recall. After intense internal tests Mares has identified a new O-ring of different material, color and hardness that can be retrofitted onto the Quick Connector of any existing Nemo Air.

We take this matter very seriously, but we would also like to point out that this issue is covered by the EN250 norm, which requires a 0.3mm opening in the high- pressure hose fitting. As a result, the hose is designed to ensure a slow enough gas release; which should allow the diver to complete a safe emergency ascent.

NONETHELESS FOR THE PERSONAL SAFETY OF YOUR CUSTOMERS AND TO AVOID POSSIBLE DANGEROUS ACCIDENTS: STOP ANYONE FROM DIVING WITH A QUICK CONNECTOR SUBJECT TO THIS VOLUNTARY RECALL UNTIL THE EXISTING O-RING IS REMOVED AND SUBSTITUTED WITH THE NEW GREEN COLOR ONE.

THE CURRENT O-RING FROM THE NEMO AIR QUICK CONNECTOR (O-RING IS BLACK OR BROWN) MUST BE REPLACED ONLY WITH OUR NEW O-RING THAT IS A DIFFERENT MATERIAL, HARDNESS AND GREEN IN COLOR.

Affected products and codes are:

Finished goods

414158 – DIVE COMPUTER NEMO AIR

              414159 - DIVE COMPUTER NEMO AIR W/COMPASS

Spare parts

44200771 – HP HOSE W/QUICK CONNECTOR NEMO AIR

44200770 – QUICK CONNECTOR ASSY. FEMALE NEMO AIR

Please contact an Authorized Mares dealer/Service Center in your area to schedule the retrofit of the O-Ring of your Nemo Air Quick Connector. If you do not want to perform the above service procedures, please contact our customer service department at 1-800-874-3236 for a return authorization number. You will need to provide the unit(s) serial number(s).

NEMO AIR QUICK CONNECTOR O-RING MUST BE REMOVED AND SUBSTITUTED WITH THE NEW O-RING.

Poseidon W50 Bladder used in conjunction with both the platform and BeSea model BCDs
(Added 07/13/2008)

Poseidon Issues Product Safety Notice - Recall (Official Announcement)

Potential defect discovered in certain Poseidon BCD Models

A potentially serious defect has been detected in a certain production run of the Poseidon W50 Bladder used in conjunction with both the platform and BeSea model BCDs.  This flaw deals with a weld in the internal air cell and it is possible that this weld could fail during a dive.  Should this occur, the BCD will loose its ability to hold air and provide flotation and buoyancy control during a dive without warning, putting the diver at significant risk of serious injury or death.

Both Poseidon Diving Systems and their US distributor, Waterproof Gear, Inc., are taking rapid action to correct this problem without any cost to the consumer.  This defect only involves bladders identified by the lot #5445.  The lot number is located on a tag permanently attached to the internal air cell beneath the zipper closure of the wing.  The lot number is a 4 digit number located at the bottom of the tag to the left.  Please see the graphic, and note the lot number underlined in red.  These bladders did not enter the market until September 17, 2007, and any bladders sold prior to that date are not impacted and do not need to be checked.  All bladders that meet the above description should be considered a serious safety risk to the diver, should not be used under any circumstances, and should be immediately returned for replacement.

Although not every bladder in this lot is adversely effected by this defect, there is no way to tell which bladders are impacted.  Therefore, every bladder identified from lot #5445 should be returned to an authorized dealer for replacement.

Consumers who may have this product are directed to return the product to their local authorized Poseidon Dealer immediately.  Do not dive with this product and do not attempt to analyze or repair any potential defect in the product yourself.  The product must be returned for identification by an authorized technician who will return any affected units to the Poseidon Distributor for repair.  .  If you have one of these W50 wings and do not have a local authorized Poseidon Dealer, contact our customer service department for directions on returning your product for repair at 727-944-5675.  All repairs will be completed without cost to the consumer.

Dealers should return the entire W50 wing assembly to Waterproof Gear, Inc. for replacement of the internal air cell.  There is no need to return the harness assembly or any other parts of the BCD.  The wings should be clearly tagged with the dealer’s identifying data and the customer’s name as the external portion of the wing will be returned to your customer.  The dealer should check to verify that the wing meets the description above before returning it for replacement.  Once you have identified that you have an affected product, box the product for shipment, and contact our customer service department to arrange for pickup of the product.

Poseidon and Waterproof Gear regret this unfortunate but unavoidable incident and wish to assure our consumers that safety is our paramount concern.  We constantly monitor our products and we have aggressively moved to rectify this situation as soon as it became apparent.  WE ARE CO M M ITTED TO PROVIDING OUR CONSU M ERS SAFE PRODUCTS WHICH ALLOW THE M TO M AKE THE M OST OUT OF EVERY DIVE.  If there are any questions or concerns, please do not hesitate to contact us.

Oceanic Announces Regulator First Stage Recall/Retrofit
(Added 01/05/2008)

Oceanic has been notified by its supplier (ROMI, Ent.) that they have received isolated reports of DX type Regulator First Stages exhibiting uncontrolled flow of air to the Second Stage.

Evaluation has shown that a specific batch of HP (High Pressure) Seats contained material that can deteriorate and fail to properly perform its mechanical sealing (shut off) function, causing an uncontrollable free flow that could result in serious injury to or death of the user.

In cooperation with the U.S. CPSC (Consumer Product Safety Commission), ROMI is initiating a voluntary recall of all affected CDX5 and FDX10 (diaphragm) type First Stages.

The affected First Stages include those that were shipped to Oceanic retailers from the factory between May 1, 2006, and Nov. 15, 2006. Other model DX type First Stages, such as the CDX, DXi, DX3, DX4 and TDX5, that received annual service between May 1, 2006, and Oct. 22, 2007, may also be affected.

NOTE: Due to the random nature utilized to assign SNs (serial numbers) to First Stages, there is only a small quantity of affected units within the large range of SNs. Since the extent of listing the actual SNs of all affected units would be impractical and not user friendly, Oceanic has created a data base on the Oceanic Worldwide web site that product owners and retailers can easily access to determine whether a specific First Stage is affected and if it has received the retrofit.

Determine whether your fist stage is affected.

Locate an Authorized Oceanic Dealer closest to you.

Instructions for returns direct to the factory.

If you have an affected unit, you must discontinue further use of it and take it to the Oceanic dealer where you purchased it or a local authorized Oceanic service center for a component retrofit at no charge.

Should you have any questions, please contact our toll-free recall help line at (888) 636-9390 or locally at (510) 639-5448 between 8 a.m. and 5 p.m. Monday through Friday, Pacific Time.

If you no longer own the DX (diaphragm) type Regulator First Stage, please forward this letter to the new owner or have them contact us directly.

WARNING: DO NOT CONTINUE USE of the Regulator First Stage until it receives a retrofit. Only authorized Oceanic dealers and their trained service technicians must perform this retrofit. Nonauthorized persons who attempt the retrofit procedure risk voiding the product’s warranty and rendering the product dangerously unsafe for scuba use.

Aqualung SOS Recall
(Added 10/23/2007)

It has come to our attention that some S.O.S. units (PN 394157] and 394057) sold since Sept 2002 may have a faulty component.
The component is known as an "elbow," and is used to transfer air from the BC to the safety tube. Some elbows have
exhibited signs of degradation leading to cracking and/or breakage.
If this were to occur, air may not successfully transfer from the BC to the safety tube, thus preventing the tube from inflating.
Furthermore, if a diver were inverted, making the attachment point the "high spot," air could also escape from the BC.
The leak may prevent the BC from "fully inflating." Although the BC will still
hold air when vertical, some air will
escape at the bottom and the BC will provide less buoyancy.
To date, there have not been any reported accidents or injuries.

Aqua Lung would like all owners of S.O.S. units to carefully inspect them. Elbows that have the potential to degrade are easily
identified by a molding gate mark located
on the outer bend (see photo, right.)

If you have a unit with such a gate mark, we ask that you stop using the product.  Detach it from the BC and return it to your nearest Aqua Lung Dealer. You may find your nearest Aqua Lung Dealer by using the dealer locator at
www.aqualung.com. Your dealer will return it
to Aqua Lung who will replace the unit at no
charge.

Should you have any questions, please call Aqua Lung Customer
Service at 877-253-DIVE (3483) or (760) 597-5000 outside the U.S.

Innovative Scuba Concepts Product Recall
(Added 06/07/2007)

Innovative Scuba Concepts Recalls Diving Regulator Components Due to Drowning Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. Swivel for a Scuba Regulator

Name of product: Swivel for a Scuba Regulator

Units: About 170

Importer: Innovative Scuba Concepts Inc., of Colorado Springs, Colo.

Hazard: The swivel, which is attached to a diving regulator, could separate while diving. This will result in a complete and sudden loss of the diver’s air supply, causing the diver to engage in emergency ascent procedures. This poses a risk of decompression sickness due to rapid ascent, and air embolism or drowning if the diver panics or the emergency ascent procedure fails.

Incidents/Injuries: The firm has received one report where the swivel separated during a dive and caused the diver to ascend using the buddy breathing technique. No injury was reported.

Description: The recall involves the HO110 Swivels sold as an aftermarket regulator component. There are no markings such as a date or production code embossed on the unit. Contact Innovative Scuba Concepts for information on where the recalled units were sold.

Sold at: Specialty retail dive stores nationwide from January 2006 through March 2007 for about $40. They also could have been installed by a dive shop regulator technician.

Manufactured in: Taiwan

Remedy: Consumers should stop using regulators with the swivel attached and contact the dive store where purchased for a refund.

Consumer Contact: For additional information, contact Innovative Scuba Concepts Inc. at (800) 472-2740 between 9 a.m. and 5 p.m. CT Monday through Friday, or visit the firm’s Web site at www.innovativescuba.com

Name of Product: Scubapro® MK 20 First Stage Regulators
Units: About 48,500 units
Manufacturer: Scubapro® USA, of El Cajon, Calif.
Hazard: Over-tightening of the Yoke or Din retainer during annual servicing of these regulators could result in a stress crack and failure of the regulator. If this occurs during a dive, air supply could be interrupted, posing a drowning hazard.
Incidents/injuries: No injuries or underwater incidents have been reported. Scubapro® USA has received eight reports of units that failed after being serviced.
Description: The recall involves first stage regulators that are marked with the Scubapro brand and identified as the MK20.

SCUBAPRO is conducting a voluntary recall of the X650 second stage regulator.
(Added 10/09/06)

A manufacturing error could cause the main housing of some regulators to change shape over time, and the cover and diaphragm to become dislodged. If this occurs, air flow will be interrupted and the regulator will no longer function.

Only X650 second stage regulators purchased from authorized Scubapro dealers from November 2004 through May 2006, are affected by this recall. You can determine if you have one of these units by looking for a visible raised bump located on the right side of the regulator between the letter “C” and where the mouthpiece attaches to the regulator body.

At Scubapro, safety comes first and we ask that you stop using the X650 second stage regulator immediately. Replacement Program

SCUBAPRO will replace your X650 regulator with a new one within thirty (30) days or sooner if your regulator is found to be one of those affected by this recall . In order to obtain a replacement, register on line by filling out the form below. After we obtain your name, address, and telephone number, Scubapro will send you a return shipping box and UPS shipping label. All you will need to do is to place the X650 regulator in this box, seal the box, attach the UPS label, and drop it off at any UPS shipping location. Once we have received your regulator, we will begin processing your replacement order.

Suunto Issues Important Computer Safety Alert (Added 06/20/06)

IMPORTANT SAFETY NOTICE REGARDING D9 AND D6 DIVING INSTRUMENTS

(DNW) - Suunto Oy today announced an important safety notice regarding its D9 and D6 diving instruments. This global decision involves D9 and D6 instruments that have been delivered to distributors and retailers, as well as product that has been sold to consumers.

Quality and safety are paramount concerns for Suunto Oy. Our testing program for the D9 and D6 product by Suunto employees and independent divers all over the world has been extensive. Despite this, Suunto Oy has identified a software bug in the D9 and D6 instruments.  The software bug may cause the D9 and D6 to incorrectly track dive time on rare occasions, potentially causing a risk to the diver.

The products affected are:

D9 serial numbers 62102582 and below

D6 serial numbers 62103693 and below

The software bug may activate when the D9 and D6 automatically recalibrate to keep the correct time, or the user manually updates the seconds in set mode. Suunto approximates that there is 0.6 % (a six-tenths of one percent) chance that the software bug may appear during a dive. When the dive time is incorrect, every fourth second is not registered in the dive time and surface interval time, i.e., dive time goes 25% slower than actual time and tissue loadings are recorded at 25% less than actual loadings.   As a trained diver you understand that this could affect your decompression requirements and that the length of a dive is one of the key factors in determining those requirements.

Even though the D9 and D6 will function correctly approximately 99.4% of the time during a dive, Suunto wants to eliminate the very small possibility of recording the dive time incorrectly. Therefore, Suunto is requesting that all D9 and D6 shall be returned for a software update that eliminates this issue. To date, there have been no known injuries related to this issue.

Action that YOU should take

·         Suunto strongly recommends that you should not use the D9 or D6 for diving without backup instrumentation before the unit has been updated with the latest software.  Please bring your D9 and/or D6 to your nearest local authorized Suunto retailer for the software updated as soon as possible.

·         Please contact your local authorized Suunto retailer – for contact information please visit our web site www.suunto.com  You will then be advised on how to bring your D9 and/or D6 to the retailer for a software update. Once the retailer receives your D9 and /or D6 they will then forward your unit to the Suunto Service Center for the software update. You will be advised by the retailer the approximate turn around time that the process will take.

·         If that is not possible or if you require any further details, contact the Suunto Help Desk at SuuntoD9-D6@nordictelecenter.fi  To contact Suunto by phone, call toll free 1 800 543 9124 in the USA and Canada, or +358 284 1160 (international call rates are applicable) from all other countries.

·         On receiving your D9 and /or D6 back, you will notice that there will be a small white dot on the rear. This identifies that the unit has been updated. As a complimentary service Suunto will at the same time replace your battery and perform a pressure test free of charge.

Suunto apologizes for any inconvenience and we thank you for your co-operation.

Cressi-sub Buoyancy Compensators (Added 02/28/05)

Cressi-sub, in cooperation with the United States Consumer Product Safety Commission, is advising Consumers to stop using Cressi-sub Buoyancy Compensators with model numbers J107, J113, J115, J119 (the numbers appear on the top right hand corner of the unit on the shoulder), immediately unless otherwise instructed. Slow leakage from the shoulder exhaust caused by expansion of an internal cable housing may result in slow deflation. This could result in unexpected buoyancy problems posing a potential risk to the safety of the diver. Although no incidents have been reported the company is nonetheless issuing a product recall for the above model numbers.

Buoyancy Compensator devices provide buoyancy control for scuba divers by allowing them to inflate or deflate their devices, thus changing the diver's buoyancy. A buoyancy compensator is a bag that inflates and deflates depending upon your buoyancy requirements. In this way, the buoyancy compensator allows you to maintain neutral buoyancy, which is an important part of scuba diving. The units were sold from January, 2003 until approximately September, 2004 and sold at maufacturer's suggested retail prices between $400 and $700.

Some of the devices with the above model numbers have already been retrofitted. To determine whether your device needs to be retrofitted look for a black retaining ring on Buoyancy Compensator devices. The ring surrounds a yellow Inflate button on the inflation mechanism and can be found adjoining a gray cover assembly (bypass case) which screws onto the inflator. Devices which have already been retrofitted, and which therefore do not require corrective action, are those with yellow retaining rings and black cover assemblies.

You can return the product to a Dive Shop Authorized to sell and service Cressi-sub products ("Authorized Dive Shops"), such as the shop from which you may have purchased the product, to have the product retro- fitted with a replacement hose at no charge to you. Returns for retro- fits should be sent to Authorized Dive Shops. For a list of Authorized Dive Shops nearest you please visit www.cressisubusa.com. Consumers can call 1- 800 338- 9143 for details Monday through Friday, 9:00 am EST to 4:30 pm EST.

Dacor Darwin Air and Darwin Console Dive Computers (Added 12/13/04)

If you have recently purchased a Dacor Darwin Air or Darwin Console Dive Computers please read the following safety notice regarding the recall of certain computers.

HTM Sports S.p.A., Manufacturers of Dacor Diving Computers, has discovered that during production, a small number of Dacor Darwin Dive Computers were improperly calibrated, resulting in erroneous calculations displayed regarding "No Deco Time", "Deco Time", "Air Time", Desat Time" and "No Fly" times.

The specific products that may have this defect are in the following groups:

Darwin Air Dive Computers Serial Numbers between: 3265 and 3415

Darwin Console Computers Serial Numbers between: 8503 and 8592

To locate the serial number of your Darwin Air or Darwin Consol:

1. Find the watertight batter compartment cover located on the top of Darwin Air or the left side on the Darwin consol.
2. Unscrew the watertight cover by turning 90deg counter clockwise
3. Remove the batteries and the put them back in after a few seconds the serial number of the instrument will appear on the screen.

If you own one of the Darwin Computers with a serial number listed above DO NOT USE IT!

Diving with an improperly calibrated dive computer will provide inaccurate No Decompression (no stop) Time, Decompression Time, Desaturation Time, and No Fly Time, and could result in serious
injury, including Decompression Sickness due to Omitted or Insufficient Decompression!

Immediately take your dive computer to your Authorized Dacor Dealer for repair. You should expect to receive it back in 4 - 6 weeks.

Dacor regrets and inconvenience cause by this recall and thanks you in advance for your cooperation.

Seaquest/Aqua Lung BCs with Powerline Inflator (Added 12/13/04)

Airway Ribbed Hose Replacement
Aqua Lung has received reports that some of the ribbed hoses on the Powerline Airway have been tearing at the top near the dual exhaust valve. The problem has been traced to an error in the molding process from our supplier that occurred over a three-month period between 2001 and 2002. The best way to confirm whether or not you have one of the questionable hoses is to look at the serial number on the lower unit of the Powerline. If the serial number falls within the range of M48H to M52H (M48H, M49H...M52H), or M01I to M08I (M01I, M02I...M08I) and is the original hose, then it should get replaced. This affects both SeaQuest and Aqua Lung BCs utilizing the Powerline inflator.

If the ribbed hose were to tear near the top, the BC would not be able to hold air and a loss of buoyancy would occur. This could result in the need for the diver to ditch their weight pockets or rely on their buddy for assistance. In extreme cases it could result in result in panic, injury or death.
If you have a spiral hose that falls within the serial number range, please have the hose replaced by this authorized Aqua Lung retailer.

Oceanic Reliant B/C Inflator Recall

Oceanic has been informed by its supplier that they have received isolated reports of BC inflator mechanisms exhibiting continual flow of air into the BC after release of the power inflation button. Affected are Oceanic Reliant model inflators that are fitted on various models of Oceanic's BCs that were sold between February 1, 2004, and June 24, 2004. BCs sold prior to February 1, 2004, do not have these inflators and are not affected.

Testing and evaluation has shown that the power inflator button may stick in the depressed position. Engineers have designed new replacement parts that will prevent the button from sticking.

Oceanic asks that all owners of Oceanic BCs that have Reliant inflators return them to an authorized Oceanic Dealer for a retrofit at no charge.

WARNING: DO NOT CONTINUE USE of the BC or attempt to disassemble any components of the inflator. Take the BC with inflator to your nearest authorized Oceanic Dealer for inspection and service. Only authorized Oceanic Dealers and their trained service technicians must perform this retrofit. Non-authorized personnel who attempt the retrofit procedure risk voiding the products warranty and rendering the product dangerously unsafe for normal scuba use.
